Refining the Prompt Forger in Real Time

Tool Used: Prompt Forger (original version)

Situation: Extended medical crisis involving cognitive decline, medication errors, and active interference from a family member. The original Forger would dictate "call 911" for every entry—useless in a 6-month prolonged situation.

What Happened: Over months of daily use, the Forger was trained to recognize the difference between a true emergency and a manageable event. It learned to calibrate responses: observation before intervention, strategic withdrawal when appropriate, and action only when safety required it.

What Worked: The final version is not the same tool that started. It was shaped by the crisis it was designed to manage. Direct Observed Dosing was implemented. The Log proved its value as evidence. Strategic withdrawal preserved caregiver energy.

What Didn't: The original "call 911" reflex had to be unlearned. The tool needed human judgment to become useful.

⚔️ Hostile Territory Protocol

December 2025 • Medical Crisis

Field Test: Active Interference During Post-Operative Care

Tool Used: Hostile Territory Protocol

Situation: Patient (70s male) post-laparoscopic cholecystectomy with severe post-operative delirium. Primary caregiver had no legal authority. Secondary actor actively interfered—medication sabotage, sleep disruption, information withholding.

What Worked:

  • The Log: Every phone call timestamped, every contradictory statement recorded. When disputes arose, the log was the deciding factor three times.
  • Direct Observed Dosing: Implemented after double-dose event with respiratory depression. Prevented further errors.
  • Pre-emptive Framing: Scripted responses ("I am following the doctor's written schedule") shut down manufactured drama.
  • Yellow Line/Red Line: Clear stop conditions prevented caregiver burnout during the 6-week active crisis.

What Didn't: Attempting to reason with the interfering actor. Logic was ineffective against patterned behavior. Strategic withdrawal worked better.
